WHO DEVELOPS MY FAMILY MEMBER’S PLAN OF CARE? It’s a collaborative effort. PLANCO staff work with clients, their families and/or legal representatives to develop a comprehensive Plan of Care that caters to the client’s unique situation and individual needs. Family direction is crucial to ensure the Plan of Care covers all the client’s present and future needs. The Plan is then implemented as written. It can be changed as needed and is updated annually. CAN I MAKE CHANGES TO THE PLAN OF CARE? Yes. The Plan of Care is flexible. It can be changed as needed and is updated each year. MY SON ALREADY RECEIVES SERVICES FROM MRDD AND/OR OTHER PUBLIC HEALTH AGENCIES. DO I NEED PLANCO? Public agency services, while helpful, may not be sufficient for the future care of your family member. PLANCO partners with public and other private agencies to oversee the continued care of your loved one. Providing some services and monitoring the delivery of others, PLANCO eventually takes on the role of the primary care giver when the family is no longer able to do so. MY DAUGHTER HAS AGREED TO CARE FOR HER BROTHER WHEN I’M NO LONGER ABLE TO. HOW CAN PLANCO HELP? Caring for a disabled family member can be an overwhelming task. Love and good intentions often collide with the reality of the demands of care. PLANCO offers assurance that the care your loved one needs will be implemented and monitored into the future. We work with siblings or other family members so that they can remain involved in the care of a disabled family member without having the responsibility of arranging for and delivering services. WHEN DO PLANCO SERVICES BEGIN? NOW, OR WHEN I’M NO LONGER ABLE TO CARE FOR MY DAUGHTER? Optimally, PLANCO staff begin to work with and build a trusting relationship with your family member now, providing at least limited services. This makes for a smoother transition of care when the family is no longer able to provide primary care. I’D LIKE TO TAKE A VACATION WITH SOME FRIENDS, AND I MAY BE GONE FOR SEVERAL WEEKS. CAN PLANCO INCREASE CARE AND MONITORING FOR THAT PERIOD OF TIME? Yes, PLANCO can provide whatever services your family member needs while you’re away, for whatever length of time. A meeting to adjust the Plan of Care is all that’s necessary. Monitoring and services can also be increased during times of crisis or anytime clients require additional support. I MAY BE MOVING, OR POSSIBLY JUST SPENDING WINTERS IN FLORIDA. HOW WILL THIS EFFECT PLANCO SERVICES FOR MY SON, WHO WILL REMAIN IN OHIO? As long as your son remains in the PLANCO service area, (Franklin, Delaware, Union, Pickaway, Fairfield and Licking counties) there will be no problem continuing PLANCO services. Communication between out-of-town family and PLANCO staff can take place over the phone and on visits to Ohio. HOW DOES PLANCO WORK WITH SPECIAL NEEDS TRUSTS? Quite easily. PLANCO supplemental services can be billed to a standard special needs trust, as well as trust funds set up for future services. Trusts are an excellent way to ensure finances are available for the future needs of disabled individuals. DOES PLANCO PROVIDE ALL SERVICES DIRECTLY? PLANCO provides many of its services directly, including transportation, life skills assistance, and social and recreational opportunities. We oversee other services that are provided by existing public and private agencies, including housing, employment and health care.
