Plan Of Care





	
PLANCO provides coordinated care and support through a 
personal and comprehensive “Plan of Care” that is developed 
with input from each client’s family or guardian. It incorporates 
all the client’s current and future needs, including existing 
benefits and services provided by other private and public 
agencies, and is implemented and monitored by PLANCO staff.

PLANCO works in tandem with these public and private 
agencies, overseeing the care they give, and providing 
services they don’t, such as escorted transportation to and 
from appointments, social and recreational activities and 
advocacy. The Plan of Care is updated annually or as needed.
