
Programs
PLAN of Central Ohio offers an activities program where participants can build friendships, strengthen social skills, and enjoy enriching experiences. We design our schedule around the interests of our participants and provide transportation to and from each activity. Our approximate 1‑to‑5 case manager–to–participant ratio helps ensure a safe, supportive, and engaging environment. Activities include shared meals, community outings, social events, educational opportunities, holiday gatherings, and both small‑ and large‑group special‑interest programs.
Advocacy
We can accompany individuals to important appointments or meetings to help them feel supported and informed. This may include appointments with Social Security, county agencies, or other service providers. Our role is to help ensure they understand the process and feel confident participating in decisions that affect them.
1:1 Phone & In-Person Connections
We offer regular individual check‑ins—either by phone or in person—to provide connection, encouragement, and ongoing support tailored to each person’s needs and preferences.
Family Surrogacy & Companionship
PLAN develops, implements, and monitors individualized care plans for people who have lifelong disabilities or long‑term support needs. Our approach emphasizes dignity, autonomy, and meaningful relationships.
Linkage to Community Resources
When a need arises, we help connect individuals to community resources and services throughout the city. Our goal is to expand access to opportunities that promote well‑being, independence, and community inclusion.
Monitoring & Supportive Assistance
When appropriate, we can provide supportive oversight in areas such as medication reminders, medical coordination, grocery shopping, and other daily‑living needs. Our focus is on promoting safety, independence, and quality of life while respecting each person’s abilities and choices.
Group Activities & Socializing
PLAN of Central Ohio offers a welcoming and inclusive group‑activities program designed to foster connection, confidence, and community belonging. Participants can explore shared interests, build friendships, and enjoy meaningful social experiences in a supportive environment. Activities may include group outings, creative workshops, recreational events, community engagement opportunities, and seasonal celebrations. Each gathering is structured to encourage choice, autonomy, and positive social interaction, helping individuals strengthen life skills while having fun and feeling part of a community.
